The Admission Process
How to Get Started
Admissions to Carolinas ContinueCARE Hospital at Pineville are accepted upon referral from a physician. All patients are admitted without regard to race, color, creed, religion, national origin, disability, age, sex, or payment classification.
When you arrive, we will strive to make you and your family comfortable. You will receive an orientation to your room and the hospital. Your nurse will conduct an interview and ask questions regarding your medical history and medications that you are currently taking. Try to answer all questions accurately. This information is essential for your medical record and plan of care.
Who We Treat
Our facility is licensed under Medicare as a long-term acute care hospital, providing care for critically ill and medically complex patients who require a longer acute care stay than a traditional hospital. There are several criteria patients must meet for admission eligibility.
- Require acute care services as determined by a physician assessment
- Cannot be treated effectively at a lower level of care
- Have multiple acute complexities that require daily intervention by a physician
Patients requiring the following medical services may also eligible for admission:
- Mechanical ventilation for respiratory failure
- Pulmonary hygiene
- Tracheotomy with respiratory insufficiency
- Exacerbation of chronic obstructive pulmonary disease (COPD)
- IV and oral anticoagulation/therapeutic INR (international normalized ratio)
- Post cardiac surgery with acute care needs
- External wound infection management
- Stage III or IV decubitus ulcer care
- Complex medical conditions
- High-level orthopedic conditions
- Pre- and post-organ transplant care
- Low-tolerance rehabilitation
Free visitor and patient parking is available at the front of the hospital.
The coordination of your care and discharge planning involves you, your family/significant other, your physician, and other clinical staff members. A Case Manager is assigned to every patient to oversee this process. Your Case Manager will visit you to discuss your care needs after admission.
At Carolinas ContinueCARE Hospital at Pineville, our goal is to meet your service needs. Your opinions and suggestions are important to us, and we encourage you to provide feedback when we meet your expectations and when you notice areas for improvement.
Members of our leadership team will visit periodically and inquire about your experience. During these visits, please share any accolades or concerns you have. At discharge, you will receive an HCAHPS (Hospital Consumer Assessment of Healthcare Providers and Systems). We encourage you to take the time to complete this survey. Your comments will be confidentially reviewed and addressed by our patient care and management teams for improvements when necessary.
